Scientific Review Paperwork
PARTICIPANTS: It is very important you read through the documents on this page thoroughly. We recommend printing them out and highlighting information specifically applicable to you. All information you need to know is in the documents below. Additional questions can be sent to the Director.
Lower Division (grades 6-8)
Scientific Review Committee (SRC) forms should be completed BEFORE experimentation begins and kept until registering for ISEF. The Scientific Review Committee will notify you if any changes need to be made. All projects involving Human Subjects, Vertebrate Animals, and/or Potentially Hazardous Biological Agents MUST fill out this form.
Scientific Review Form – Middle School
If you are including human participants you must also fill out the Informed Consent Form with information about your experiment and then print out a copy for each participant that they can sign physically. You do not need to send all forms in, but you need to have them with you for judging. If you have any questions regarding lower division projects email sciencefairAT randallmuseum.org.

Upper Division (grades 9-12)
GGSF follows the International Science and Engineering Fair (ISEF) guidelines and uses their forms for project approval. Scientific Review Committee (SRC) forms should be completed BEFORE experimentation begins. The Scientific Review Committee will notify you if any changes need to be made. All projects involving Human Subjects, Vertebrate Animals, and/or Potentially Hazardous Biological Agents MUST fill out the appropriate forms. If you are unsure of the forms to fill out, use the ISEF Rules Wizard to determine necessary approvals.

Important: It is your responsibility to have your forms signed and most forms should be filled out BEFORE you start a potentially hazardous experiment.
